OMD Online Testing is a restricted online testing platform for use by paramedics and EMTs who are authorized by the Office of the Medical Director (“OMD”) for use of the site and OMD Online Testing. Unauthorized entry into the OMD Online Testing is prohibited and may be punishable by law. By using the OMD Online Testing you expressly agree that all information and responses that you provide are true, honest and accurate. You also expressly represent and agree that the responses provided when using the testing platform are your own work, answer, or response and that you have not anyway collaborated, relied upon, consulted, or otherwise been provided the information for the response from any other individual(s) or source(s) which is not expressly provided for as part of the testing protocol. You are not allowed to consult your protocol manual as a resource for supplying answers to questions, and doing so would be considered dishonest behavior in completing the exam on OMD Online Testing. You are expressly prohibited from asking any other individual for an answer or any other assistance when completing the testing platform. IF IT IS REPORTED OR DETERMINED THAT ANY OF THE INFORMATION PROVIDED BY YOU WAS FALSE, FRAUDULENT OR DISHONEST THIS SHALL BE GROUNDS FOR IMMEDIATE SUSPENSION OF YOUR CLINICAL PRIVILEGES PROVIDED THROUGH OMD AND WILL BE SUBJECT TO INVESTIGATION PURSUANT TO THE PROCESSES AND PROCEDURES ADOPTED BY THE MEDICAL CONTROL BOARD. IF IT IS REPORTED OR OTHERWISE DETERMINED THAT YOU COLLOBORATED OR OTHERWISE RELIED UPON PROHIBITED SOURCES OF INFORMATION IN COMPLETING THE TEST THIS SHALL BE GROUNDS FOR IMMEDIATE SUSPENSION OF YOUR CLINICAL PRIVILEGES PROVIDED THROUGH OMD AND WILL BE SUBJECT TO INVESTIGATION PURSUANT TO THE PROCESSES AND PROCDURES ADOPTED BY THE MEDICAL CONTROL BOARD. Your privacy is very important to us, and we want you to understand our practices with regard to collecting and using your information both online and offline and the measures taken to safeguard your information. Your information will only be used for Office of Medical Director official business.If you are visiting the site regardless of whether you intend to utilize OMD Online Testing, please note that we collect and store the following aggregate information for analytical purposes: - The name of the domain from which you accessed the Internet; - The IP address from which you accessed the website. The laws of the state of Oklahoma govern these terms and conditions. By accessing this website and using OMD Online Testing, you consent to these terms and conditions and to the exclusive jurisdiction of the state and federal courts of Oklahoma in all disputes arising out of such access. If any of these terms are deemed invalid or unenforceable for any reason, the invalid or unenforceable provision will be severed from the remaining terms and the remaining terms will continue to apply.
